Name, Image, and Likeness Release

I grant Sylvia’s Sisters and its affiliates the right to use, reproduce, distribute, and display photographs/videos containing my name, image, or likeness in any media, now known or hereafter devised, including but not limited to print, electronic, and digital formats.

I authorize Sylvia’s Sisters and its affiliates to use these photographs/videos for the purpose of promoting the organization, its programs, its events, and any related activities. I understand that I will not be compensated for the use of these photographs/videos.

IF THE PARTICIPANT IS A MINOR, A PARENT, GUARDIAN, OR RESPONSIBLE ADULT MUST SIGN BELOW.

On behalf of the minor child, I accept and agree to the full contents of this agreement. I certify that I have the authority to sign on behalf of the minor child and to make decisions for the minor child regarding this activity.
